Step #3: Submit form and payment before the deadline of Friday, May 8, 2009.

If payment is being made by American Express, VISA or MasterCard you can fax your registration to 416-849-0408. Please fax both pages of the form and all required supporting documentation. Please remember to include all pages of the form and supporting materials, as incomplete applications cannot be processed.

Our Cancellation Policy
If you withdraw your application or cancel your registration before the April 30, 2009 deadline, you will receive a full refund minus a $50 administrative fee. After May 18, 2009 no refunds will be issued.
